I've been tutoring online for a long time now.
My lessons are fully personalised. Before the first lesson, I discuss with each student what he/she needs to work on, what he/she needs to improve and generally what is needed to do to get better and fluent at the language.
In my opinion, conversations and more relaxed classes are the best forms.
